The list of ICD-9 codes used to identify hospitalizations in Table 2, to identify influenza vaccination in Table 5 and identify comorbid conditions in Table 10 of the DFR. They are also used to identify infection in table S1 of the Supplement to the DFRs. Additional, table S2 in the Supplement also used V-modifiers and Value Code D5 for Kt/V.
