Q

How do I login to access my Dialysis Facility Report?
A1. From an Internet browser, go to www.dialysisreports.org
2. Click on “Secure Log-in
3. Enter your user name and password.
  • Have you logged in previously? If so, your username is your email address, and your password is one that you created yourself at first login.
  • Forgot your password? Click on “Forget your password?” and a new one will be emailed to you.
  • Don’t have a username & password?
    • If you are a facility representative: Master Account Holder username and password information was listed on the first page of your 2009 DFR. If you no longer have a copy of it or are a new facility, please contact your Network to receive your Master Account Holder credentials. Once logged in, Master Account Holders may create individual user accounts for authorized individuals.
    • If you are a Network representative: A Master Account Holder username and password was included in the cover letter that accompanied the DFRs. Once logged in, Master Account Holders may create individual user accounts for authorized individuals.
    • If you are a State Surveyor or CMS Region Representative: A Master Account Holder username and password was included in the email sent when the Region and State Profiles were made available on the Web site. Once logged in, Master Account Holders may create individual user accounts for authorized individuals. A list of State and Region Master Account Holders can be found here.

I don’t have a username or password. How do I get one?
AThe Master Account Holder for your dialysis facility, ESRD Network, State Survey Agency, or ESRD Region may create individual login credentials for you and other authorized users. Please contact the Master Account Holder to request an individual account.

I have forgotten my username and password.
AClick on “Forget your password?” (from the main secure site login page) and a new temporary password will be emailed to you.

Who is the Master Account Holder for me?
A If you are a facility representative: Each facility is responsible for designating their own Master Account Holder is but typically it will be the Medical Director or Unit Administrator. The Master Login Credentials appeared on the first page of the 2009 DFR. Facility representatives can also contact their Network to obtain their Master Account Holder Credentials.
If you are a State Surveyor or Region Representative: You can see who received the Master Account Holder Credentials here.
If you are a Network representative: Your Master Account Holder Credentials are included in the cover letter that accompanied the DFRs.

Q

I am the Master Account Holder. How do I create access for others?
ALogin to the secure site using the Master Account Holder credentials. Click on “Create/Edit Users” near top of page. Click on “Create New User”. Complete the requested information. A temporary password will be sent to the new user’s email address provided. The new user will need to log on to the Web site using the email address as the username and the temporary password received. Once logged on, the temporary password may be changed to a preferred password.

Our Master Account Holder is no longer employed here. Who do we need to contact?
A If you are a facility representative: Please refer to the answer for facility representatives under "Q. How do I login in to access my Dialysis Facility Report?... Don’t have a username & password?".
Important: You may want to deactivate the past employee’s account by editing the account information (click “Edit” and change the enable option to “No”).
If you are a Network, State, or Region representative: Please contact the Kidney Epidemiology and Cost Center by email at keccdfr@umich.edu or call 734-998-9823 to provide information about staffing changes.

My username and password aren’t allowing me to log in.
APlease be careful if your password includes one of the following: zero "0", upper case "O", lower case “L”, upper case “i” or one "1". Also, if your password is available electronically, you may want to try copying and pasting the password directly from the document.
