Q	How do I login to access my Dialysis Facility Report?
A	1. From an Internet browser, go to www.dialysisreports.org 2. Click on “Secure Log-in” 3. Enter your user name and password. Have you logged in previously? If so, your username is your email address, and your password is one that you created yourself at first login. Forgot your password? Click on “Forget your password?” and a new one will be emailed to you. Don’t have a username & password? If you are a facility representative: Master log-in information was listed on the first page of your DFR. Once logged in, master account holders may create new user accounts for authorized individuals. If you are a Network representative: A Master username and password was included in the cover letter that accompanied the DFRs. Once logged in, master account holders may create new user accounts for authorized individuals. If you are a State Surveyor or CMS Region Representative: A Master username and password was included in the cover letter that accompanied the Region and State Profiles. Once logged in, master account holders may create new user accounts for authorized individuals. A list of state and region master account holders can be found here.

Q	I don’t have a username or password. How do I get one?
A	The Master Account Holder for your dialysis facility, ESRD Network, State Survey Agency, or ESRD Region may create login credentials for you and other authorized users.

Q	I have forgotten my username and password.
A	Click on “Forget your password?” (from the main secure site login page) and a new temporary password will be emailed to you.

Q	Who is the Master Account Holder for me?
A	If you are a facility representative: Your Master Login Credentials appear on the first page of your DFR. Facility representatives can also contact their Network to obtain their Master Login Credentials. If you are a State Surveyor or Region Representative: You can see who received the Master Login Credentials here. If you are a Network representative: Your Master Login Credentials are included in the cover letter on the DFR Network CD.

Q	I am the Master Account Holder. How do I create access for others?
A	Login to the secure site. Click on “Create/Edit Users” near top of page. Click on “Create New User”. Complete the requested information. The new user will receive login credentials at their email address.

Q	Our Master Account Holder is no longer employed here. Who do we need to contact?
A	Please contact the Kidney Epidemiology and Cost Center by email at keccdfr@kecc.sph.umich.edu or call (734) 998-6835 to provide information about staffing changes.
